- Some of the questions on this form may surprise you, but the answers (used anonymously) are essential when OAV applies for grants to show that overall, OAV serves a diverse population. Please know that the answers to all questions except name, email and membership type are optional.
- Membership options: full fee dues are $200 per person per year. For those unable to pay this amount, there are two additional options of $120 and $60 per person per year.
- Once you have completed this online form, be sure to confirm consent at the bottom of this form and click on the SEND button.
- The next step in becoming a member is to go to the top right corner of any page on this website and click on the DONATE NOW button. That will take you to a page where you will scroll down to the DONATION AMOUNT box and write in the dues amount that you have chosen (the dollar sign will auto-fill). Skip the monthly donation box and then fill in your name and address, skip the fundraising box and scroll down to COMMENTS. In this box type ‘for yearly membership’. Finally scroll to PAYMENT METHOD—put in your credit card number to pay by credit or debit. If you wish to pay with something other than a credit card, please indicate this in the comment box.
- Once you have completed this form and made payment, the Village will run a background check and mail you a membership agreement for signature.
